If you are unable to complete a homework, or if you miss the midterm test, due to illness or other emergency you must fill out the Request for Special Consideration form and bring it, together with your supporting documentation, to the instructor at the earliest possible time.
In the case of illness, medical documentation must be supplied on the standard University of Toronto Student Medical Certificate . A simple "doctor's note" is not acceptable.
