• Remark Request Form There may be situations in which you feel your marker has made a mistake. Begin by discussing this with your TA, who usually can resolve the problem. If you are still unsatisfied, you may submit this form to your instructor. You must clearly explain why you feel your work was marked unfairly. (Your mark may increase or decrease based on this regrading; the entire assignment will be regraded.)
• Special Consideration Form/Standard UofT Medical Certificate If you are unable to complete homework or if you miss the midterm test due to major illness or other circumstances completely outside of your control, have your doctor complete a Student Medical Certificate, print this form, and submit it (along with your Student Medical Certificate) to your instructor as soon as you are well.
